How to use this assessment
Work through each question honestly. For each one, note whether your answer is Yes, Partially, or No. At the end, count your No and Partially answers. Each one represents a gap with a measurable cost attached to it.
This assessment is not designed to make you feel good about your current setup. It is designed to make the gaps visible so you can decide which ones are worth closing.
Most owner-led clinics score 4 to 6 Yes answers on their first pass. If you score 8 or higher, your operations are genuinely strong. If you score 3 or lower, there are likely multiple areas where revenue is leaking quietly.
The 10 questions
- 1Do you know your exact no-show rate for the last 30 days?Not an estimate. A number you could pull in under two minutes from your practice management system.
- 2Do patients receive at least three reminders before their appointment?A confirmation at booking, a reminder 72 hours out, and a day-before confirmation with an easy reply option.
- 3Do you have a system for filling same-day cancellations?A short-notice availability list, waitlist notification, or equivalent process that recovers cancelled slots within hours.
- 4Are after-hours calls handled in a way that can book appointments?Not just a voicemail. A system that can capture intent and schedule a visit without requiring a callback the next morning.
- 5Do you follow up on outstanding balances within 7 days of the statement date?Not 30 days. Seven. Recovery rates drop sharply after the first week without contact.
- 6Does your collections follow-up use at least two channels?Email plus SMS, or email plus phone. Single-channel follow-up consistently underperforms multi-channel.
- 7Do inactive patients receive recall outreach at least once every 6 months?Patients who have not scheduled in 12 to 18 months but have not formally left your practice are a recoverable segment.
- 8Can your front desk staff handle a call surge without dropping calls?If two staff members are occupied and a third call arrives, what happens to it? Do you know?
- 9Do you track new patient conversion rate separately from overall call volume?Knowing how many people call is not enough. Knowing what percentage of new patient inquiries result in a booked appointment is what matters.
- 10Is the time your front desk spends on routine tasks declining year over year?If your practice is growing but your administrative burden is growing at the same pace, something is wrong with the leverage in your operations.
Interpreting your results
Every No or Partially answer has a dollar figure attached to it. No-show gaps cost most clinics between $3,000 and $15,000 per month in lost production. After-hours call gaps typically represent 15 to 25% of new patient inquiries that never convert. Collections gaps compound over time as older balances become harder to recover.
The value of this assessment is not the score. It is knowing which gaps exist so you can prioritize the ones with the highest return on resolution.
